You just need to provide ff requirements

CBC, blood typing, urinalysis, HBSag, VDRL, HIV tes,
Transvaginal ultrasound (24 weeks and below results only)

Original & 1 photocopy

available monday to friday 7-830am

You'll have your initial interview and first check up on the day na ipasa mo un requirements.

About naman sa hospital charges, pregnant women will get fixed 70% discount on lab and delivery itself. Check ups are free :)

In my case, I gave birth with my daughter last April 2016 via C section. The total hospital charges I paid is around P14,000 only (it also include my daughter's hospital bills also like newborn screening). Philhealth discount is already deducted.

The good thing is your child can also have their check ups at SLMC after giving birth, they will also give your baby free vaccines.

Hope this will help :)
