Vision and Hearing Screening Test: P200
Procedure: Secure payment slip from registrar's office upon presentation of birth and/or baptismal certificate. Pay application fee and vision and hearing screening test fees. Present the receipt of payment and submit the application form, health record form and "The XS Brochure." Submit all requirements for approval to the registrar's office. Proceed to the testing and research center; present proof of completion and claim the Nursery Admission Test Schedule Slip. Successful applicants will get a call from the office and be issued application forms. Bring the applicant to the TESTING & RESEARCH CENTER (TRC) on the scheduled Group Assessment and Individual Testing date. Make sure to bring the Nursery Admission Test Schedule Slip for entry to the school and for the Group Assessment and Individual Testing. Results of application will have to be picked up at the TESTING AND RESEARCH CENTER (TRC). They will not be sent to you.
