Requirements for Enrollment:

Grade 1

• Original PSA Birth Certificate
• Kindergarten Certificate of Completion for children who have completed Kindergarten under DepEd schools and centers
• Result of PEPT or PVT for children who have completed Kindergarten in non-DepEd schools and centers or home-schooled learners

Grade 7

• Original PSA Birth Certificate
• SF 9 Grade 6 (formerly Form 138) for Grade 6 Graduate
• Result of PEPT or A&E Test (for PEPT or A&E test passer)

Grade 11

• Original PSA Birth Certificate
• SF 9 Grade 10 (formerly Form 138) for Grade 10 Completer
• Result of PEPT or A&E Test (for PEPT or A&E test passer)

In addition to these minimum documentary requirements for each key grade level, the Basic Education Enrollment Form will also be required for all incoming Kindergarten, Grade 1, Grade 7, and Grade 11 enrollees and transferees at public elementary and secondary schools.

In the absence of an Original SA Birth Certificate, a Birth Certificate (late registration) from the local civil registrar or a barangay certification must be submitted by the parent or guardian
